Typically everyone has the add on price the same as the buy in and rebuys.

There's a reason that they do- the chips are worth less at that stage of the tourney, due to higher blinds and more chips in play.
I'd suggest rebuys at T1500 and add-ons $10 for T2k or $15 for T3K-4K.
