I have a box at the Borgata and I don't know the answer to any of your questions.
If I remember correctly, I paid $40 for a year. I paid all at once, no monthly fees.
As far as cutting a check, the safe deposit box should make no difference. I've never had a check cut at a casino, but I'm sure that would be done at the cashier, not through the safe deposit box people.
