I think that the only concern should be the long term health of the player. If the doctor says that playing presents an undue risk of creating long term health problems, then that player should not play. Period.

When dealing with brain injuries, the consequences extend well beyond the player's career. Since players will almost always say that they are willing to play regardless of the consequences, the decision should be taken out of their hands.
