I requested a check from River Belle yesterday (Prima). After you make a withdrawal request there is a link to hit for other options. You eventially get to check and fill out the form. I actually called into River Belle to figure this out and customer service requested I e-mail them a utility bill with my current address and a copy of my license to process the request.
