Default Voluntary statistics survey.

So I've seen a zillion posts asking questions about appropriate levels for various Poker Tracker statistics. The typical response is "it doesn't matter, anything works, concentrate on play, ignore the statistics, yadda yadda yadda." About a week ago Grunch suggested (here) that "someone" should take up a survey to figure out if there are any common trends in the Poker Tracker statistics for long-term winning players. A similar survey was done in the small-stakes limit forum about a year ago, and the results were interesting to most newer players and many long-term players.

So, I've decided to take up Grunch's cause and do a survey.

If you are interested in participating, here's what I request:

1. Check your winrate. Fill in your statistics and type in 94% for confidence interval. If your "Minimum BB/100" is greater than zero, you're safely a winning player.
2. Use Poker Tracker to gather the statistics below.
3. Fill out the form posted below and either post here or PM me with it if you'd rather not make your statistics public.
4. Please keep your data separated by stakes. If you're a proven winning player at more than one level, feel free to make separate posts/PMs about each. (Gah, this is going to suck for me.)

I'll gather it all together and make a "statistics of winning NL players" post similar to the one made by CallMeIshmael in the limit forums a year ago (here).

I've broken the requested data up by where it can be found in Poker Tracker. This is totally voluntary, but the more data I get the better the results will be.
