Q2 This depends on what site you play on. HU at FT, Stars and UB(?) have rake capped at $0,5. At $0.5/$1 NL HU you will rake approx $50 per 1000 hands...

fwiw I think it's more likely for villain to be folding a mid pair to a flop c/c and a turn c/r than a flop raise. Sometimes we miss out on the occasional turn bet with air though.
