Wells Fargo (min $25k) and Zecco (min $2.5k) both offer free trades. Bank of America does as well, but BofA requires $25k in checking/low-interest savings which makes the deal worthless.

This is very possible. ING may charge you a small fee, however.

Typically, you just have to fill out transfer forms for the receiving company (Vanguard, in your case), and they will do the rest...

VTI is the best of those four funds. It's well diversified and cheap. The others are redundant. I would also suggest to diversify into other asset classes, such as foreign stocks, to reduce your risk.

I doubt you will beat the market. Most people don't.

I see no reason at all to take on unrewarded, diversifiable risk by owning only a few stocks. This will hurt your risk-adjusted returns.
