What would raising the river accomplish? I don't think a good thinking player is donking into 2 opponents, who have shown no indication they're folding, with the intention of folding to a raise.

If you read villain as being passive just call the river raise. Many straight and flush combos just got there, and he is obviously not afraid of that.
I think everything else is fine.

Please raise preflop, CO is exactly the type of player you want to isolate. You say you're worried about him being showdown bound, but your hand has showdown value and you should have a big edge...

I agree with mvoss that you have very little fold equity. There are so many draw combos that villain can put you on, and you don't have many outs to improve.
