Is it a players responsibility to know exaclty how many chips the other player in the hand has before he says "all-in" or should all players have their biggest denominations in plain view out in front?

Depends on where you play, but commonly it's required to keep your big chips visible to the rest of the table. It's also quite bad form to intentionally hide your high denomination chips at the back of your stack.

That said, if you are unsure of how much the other player has it is always better to make sure by asking first.

Both.

It is considered to be angle shooting to "hide" chips and cash from plain view. Many card rooms have a rule board that explicitly forbids this. The local casinos here have recently stopped letting Cash play at the tables because people can more easily hide $100, add to their stack of $100, or "go south" with cash.

However it is also the player's responsibility to confirm the other player's stack before acting.

It is a common rule for players to have to keep their largest denom chips in the front.

I wouldn't really call it a matter of "responsiblity" for a player to keep track of other players' stacks. If a player announces all-in, he can't very well take back part of his bet if a bigger stack calls. It's just a rule and should be common sense.

Higher denomination chips must be in plain view.

I wouldn't say a player has a responsibility to "know" exactly how much an opponent has, but he should be able to look at the opposing chip stack and get a reasonable estimate. If large value chips are "hidden" behind small ones he cannot do this. Of course, one can always ask.

Both really. But if you really want to call you should just ask for the chip count of the player you are calling. Or you could just quick call and then figure it all out after the hand.
