My wife received a text message from a number that wasn't in her address book and wasn't addressed to her (i.e. it said "Hey, Becky" instead of "Hey, Betty"), and the message was essentially:

And attached was a picture of an erect black penis.

Seriously, did she show this to you or did you just happen to come across it (if you are snooping on your wife's phone, maybe you are looking for "signs" of cheating when there really are none)?

Have you talked to her about this and solicited an explanation?

Has she given you any reason to think this could be happening?
