Do they still want a scan of your latest utility bill too or do they just want the license?

License and utility bill with matching addresses. It needs to be a full utility bill too, I sent them just the front page of one of my bills and they said do it over again with the whole thing (weird).

As for not being 21, I don't think it should matter as long as you're over 18. However you may have trouble finding a utility bill at that age? They'll probably work with you as long as you have some other document(s) with your name and address (like a bank statement).

Checks may be requested for amounts $300 or greater up to a maximum of
$10,000, and can be requested only once every 7 days.

If you would like Full Tilt Poker to send you a check, Anthony, we must
first verify your account information. Please email or fax us a clear
and legible copy of your photo ID along with proof of address. The name
and mailing address on your ID must match your Full Tilt Poker account
information. We accept the following forms of ID:

Photo ID: Driver's License, Passport, Government-issued ID, or Student
ID

Address Verification: Bank or Credit Card statement, Utility Bill, Cell
Phone Bill, Home or Auto Insurance papers

You can email these documents to us at support@fulltiltpoker.com or fax
to 1-877-TILT-FAX. Please use a resolution of 400 dpi or higher when
sending your documents and make sure faxes are clear and legible or
they
will be asked to be re-sent. If you are using fax, make sure to include
your Player ID on all pages.

Once you are verified for checks, you will need to request a cashout
through the Cashier window. Approved checks are delivered by regular
mail in 25-35 business days. At this time, we regret that we are
temporarily unable to deliver checks by UPS to US residents. We are
working speedily to have this option available to US residents again
soon.

FWIW- I withdrew by check on Jan. 3 and got the check yestereday (Jan. 29). The check was cut on Jan. 11 but magically took 4 weeks to arrive. I withdrew by check again around the 15th and I don't expect that to come for weeks.

Re: Checks from Full Tilt

Full Tilt customer support rejected the first proof of address document I sent them. Their form letter did not make much sense, calling the document a utility bill (when it was in fact an insurance bill) and mentioning that it was not 'complete' (the pay stub at the bottom had been torn off).

I sent them a photo of a different document and they rejected it as well, saying it was too blurry. It was a little blurry, but entirely legible. They advised that digital cameras produce images inferior to scanners and faxes (welcome to 1993?).

My friend just had his two proof-of-address documents rejected also, both on the grounds that they were too blurry. He had scanned them at work and, to my eye, look perfectly sharp.

Is FTP summarily rejecting all proof-of-address documents? Has anyone else had this problem? Do I really have to learn what a fax machine is?

phidelt, that check request on 1/15 is probably one of the first post-chit checks. Keep us posted as to when it arrives.

They were kind of a pain about my documents, too. The biggest issue was the drivers license. I made a copy of my DL and then scanned the copy. I actually sent it by fax and by email, and one customer service person said it was acceptable and another said it was too grainy. Eventually, I just had a color scan done and they accepted it. I haven't requested any checks yet, but right now it is the only way I have to take money off the site so I'm glad the option is available.
