It has a lot to do with every other movie you've ever seen. We've learned that, especially in a scary scene, the bad guy is never where he's supposed to be. He'll pop out from somewhere, but never where they told he was. How many movies have the cliched scene where the victim slowly peaks around the corner, doesn't see anything, breathes a sigh of relief, and then gets suprised from behind? It's practically mandatory.

So in this scene, when they hit you over the head with the idea that "there's a monster right behind that corner", your brain just shuts that down as a possibility. You have no idea what to expect, but you know there won't be a monster behind the corner. And then there is.

Not so obvious. Maybe it's just because it's his latest that you rank it so highly. I like it a lot as well but I definitely cannot rank it ahead of Blue Velvet, Mulholland Drive or The Elephant Man. I'd place in a a group with Eraserhead and The Straight Story. Definitely better than Wild at Heart, Lost Highway and Dune though.

EDIT: I'm not so sure about the whole "lol - only 100K watching in the US" comment. The film never received a proper release in North America. It played some festivals and had a few special screenings but that was the extent of it.

it was one of his (if not the) most abstract movie he ever made. i like that. elephant man for example is surely a great movie but not even close to lets say eraserhead or inland empire if you ask me.

Unfortunately Netflix has yet to get the first season on DVD, and while I know I'd enjoy it, I've yet to buy it.

Mulholland Drive is a masterpiece. And it is definitely not one of those pseudo-intellectual films like Donnie Darko or The Butterfly Effect.
