Ok I looked, and I deleted my FTP database because it was huge, but I had datamined a bunch of your hands because I thought you were a good regular, and then I looked at some hands (I don't have any specific examples), and made notes on you on FTP.

Your first problem is thinking that you're good.

You have this burned into your head, so automatically you're gonna think you're better than everyone at the level and you're gonna be pissed when you aren't running good.

Well maybe you don't have any big leaks in PLAYING your hands, but you probably have huge leaks when it comes to emotional control and you end up getting sucked out on (Oh no, the world is gonna end!) and you get pissed that you don't win a pot and you end up making bad river calls and you magnify your small leaks when you get pissed.
