once
I posted a link with no msg to another website with some story. I don't remember what the link was for exactly, but with only 100 posts to my name, it was taken as spam and I got axed.

twice.
once for making fun of tilts mcfabulous (who turned out to be cinnamon wind, etc.) other was when i made fun of censored. yeah he was a great moderator.

[censored] banned me on his last day for a thread I started about another thread. It was where I urged voters in the Speilburg poll to vote for Raiders. This was a minor violation of The Repetition Rule

It was my good fortune (and your misfortune) that [censored] quit that very same day and I was reinstated as part of the general amnesty. I felt much like a murderer would feel if he was let out of prison with a bunch of shoplifters.
