...no one has yet mentioned that some of the games mentioned were dealt by the players (at least in the LA cardrooms). This was true through at least the mid-90's.

For anyone interested in a glimpse of the LA card room culture of say 25 or 30 years ago, Hayano's "Poker Faces" is a true classic.
