For me, first place by a long shot was the prison fight scene in "Runaway Train" where Jon Voigt gets stabbed in the hand. A distant second is the 500 fights scene in "Knockaround Guys".

There is a fight scene in Oldboy where the hero just kicks about thirty guys asses at once. Suprisingly, the scene is very realistic.

In fact, that whole movie is way macho. Just watch it.
