seeing as how i have made this rulling many many times, i can tell you exactly what the right ruling is. first of all when the hand is heads up cards can be exposed because there are no third party actions to worry about. second the turn and river should be shuffled back into the deck so that those cards are still live and in play.
more often then not this happens with just the turn card being dealt to early, in which case you complete the action and then deal the river card which would now be the turn card, and then after which you shuffle back the early turn card so it's still live. the reason for doing it this way is because you keep a much larger percentage of the actual hand that was supposed to be dealt. i hope this all makes sense, and i hope you made the right ruling but if not the best thing to do is just make a ruling and stand by it and walk away. rulings are final!!
