I know I have to fill one of these out...can anyone point to me which one it is?

Just in case those links don't work, go to www.irs.gov (http://www.irs.gov), in the keyword search at the top right of the page type "foreign account" without the quotes and click on the first result. That will take you to the page in the second link where the form is in another link at the bottom of the page.

Only problem is the form was due by June 30th. If you search the forum here, many were saying to simply fill in the form if late and attach a written explaination as to why you were late. You should probably talk to a professional(CPA). Good luck
