Bet/fold of course

Worse hands will check behind and you lose value, and better hands will bet, so you should either b/f or c/f and since c/f is not an option, b/f it is.

You don't think a worse hand ever bluffs at the 4th spade here?

What it comes down to is what is more likely:

A worse hand bets this river or
A worse hand calls this river
