In what increments is the Full Tilt bonus released? The conditions say "You can receive up to $18 per 100 hands," but doesn't clarify (so far as I can tell) if that means it releases $18 at a time... or a more normal $10 at a time, or all at the end, or what.

10% of the total or $20 whichever is less.
